This form is an application to join the Table Tennis WA (TTWA) State Development Squad. Submitting an application does not guarantee a place in the squad.
Applications will be reviewed by TTWA. Applicants will be advised of the outcome and, if accepted, provided with details about training times, fees and program expectations.
Squad members are expected to attend training regularly, arrive on time, follow coaches’ instructions and treat other players, coaches and staff respectfully.
Any fees and payment arrangements will be communicated by TTWA. I agree to meet the applicable payment requirements if offered a place.
I have provided accurate information and will notify TTWA of any relevant changes, including changes to my contact or medical information.
I understand that participation in table tennis involves physical activity. I will tell TTWA about any medical condition, injury or support need that may affect safe participation.
I understand that TTWA may contact me about this application and the State Development Squad.
